Overview

HPP developed a framework to define what is needed to implement system-strengthening precision cancer medicine (PCM) clinical trials. The framework was developed with a multidisciplinary group of experts and applied to an ongoing Swedish trial. These concrete findings, and the open access of the framework itself, have enabled it to inform the design of PCM clinical trials in Europe.

The challenge we faced was formidable: how to translate cutting-edge scientific, regulatory, data, economic and clinical complexity into a coherent, actionable guide that health systems could realistically absorb and adopt. HPP masterfully bridged this “translation gap”, anchoring the framework in robust evidence while keeping it practical, context-sensitive and adaptable.

The challenge

PCM is an increasingly important aspect of cancer care; however, its availability varies considerably in Europe.

Clinical trials play a pivotal role in preparing health systems for the large-scale implementation of PCM. These trials can act as testbeds, assessing everything from data collection platforms to the diagnostic capabilities of a country’s health system. But they need to be designed with the objective of building health system readiness from the start – otherwise there is a risk that the therapies will not reach the people who need them, even if they are found to be effective.

Testbed Sweden for Clinical Trials and Implementation of Precision Health in Cancer Care commissioned HPP to develop an implementation framework that could support researchers in designing clinical trials that prepare health systems for the implementation of PCM.

What we achieved

We co-developed and launched an openly licensed, modular implementation framework with a multidisciplinary expert community, creating a practical tool to support the successful translation of PCM innovations into routine care.
HPP enabled researchers to identify implementation barriers earlier, design more feasible trial protocols and strengthen pathways to scale-up, accelerating preparedness for real-world adoption of PCM approaches.
We established a shared methodology for implementation planning that has been adopted by research teams and recognised by leading European initiatives, including PCM4EU and PRIME-ROSE, helping to align approaches in the field.
We demonstrated the value and feasibility of PCM implementation through the FOCU.SE application, contributing to the successful approval and partial funding of Testbed Sweden by the Swedish Government in 2025.
